Dear Ms. Requester:

Your request to serve alcoholic beverages on DATE OF EVENT at the LOCATION has been approved by the Alcohol Review Board. Based upon the information provided on the form, your event has been found to be in compliance with state and university rules and regulations regarding the service of alcoholic beverages.

Please remember that it is a violation of New York State law to sell, serve or give any alcoholic beverage to any person under the age of 21 years of age.

Also be mindful of the following conditions which must be met in order that your event remain in compliance:

1. There is no admission charge or exchange of money at the event without a liquor license;
2. That no one under the age of 21 is served an alcoholic beverage, and that appropriate measures are taken to insure this;
3. That no one who is intoxicated is served alcoholic beverages;
4. That food and non-alcoholic beverages are also available;
5. That the area is to be cleaned and returned to its original condition after the event; and
6. That the person who has accepted responsibility for the event, is present and understands his or her responsibility for the conduct of the event.
